Cardinal Community School District has partnered with Wapello County Public Health to offer COVID-19 vaccines to students 12 years and older and flu vaccines to all Prek through 12th grade students.

Consent forms will be sent home with students and must be completed and returned to the school prior to the date that the vaccines will be administered. 

COVID-19 Vaccine

On October 15, Cardinal CSD will be hosting a COVID-19 vaccine clinic for any student 12 years old and up. For participating students, the first dose will be administered on 10/15/21 and the second dose on 11/15/21. 

This is voluntary and only will happen if parents/guardians have signed a consent form (see documents linked below). No student will receive the vaccine on campus without parent/guardian signed consent.

If you do want your child to take part in the clinic, please fill out the forms below and return them with your child ASAP so the appropriate amount of vaccine is allocated.
